BreezeMed Urgent Care is owned and operated by Health Wellness Medical Astoria PLLC. Administrative and management services are provided by Shifa Care Group LLC (MSO). The MSO is not a medical practice and does not direct clinical judgment.

Telehealth services are provided by licensed clinicians where permitted by law. You consent to receive care via audio/video technology, acknowledge the potential limitations of virtual exams, and agree to provide accurate information. We may request an in‑person visit when clinically necessary. Telehealth is not appropriate for all conditions.
