This disclosure is provided in accordance with New York State Corporate Practice of Medicine regulations to clarify the ownership and management structure of BreezeMed Urgent Care.

Ownership and Operation

BreezeMed Urgent Care is a medical practice owned and operated by Health Wellness Medical Astoria PLLC. All clinical care and medical decision-making are the sole responsibility of licensed healthcare providers employed or contracted by the PLLC.

Management Services Organization (MSO)

Administrative and management support services for BreezeMed Urgent Care are provided by Shifa Care Group LLC, which serves as the Management Services Organization (MSO). The MSO provides non-clinical services such as facilities management, human resources, billing, marketing, IT support, and other administrative functions.

Shifa Care Group LLC does not practice medicine, direct medical care, or influence clinical judgment. All clinical decisions are made solely by the licensed medical providers of Health Wellness Medical Astoria PLLC, under the direction of Olutimilehin Olutomiwa Oyeniran, MD.

Purpose of Disclosure

This disclosure is intended to provide transparency regarding the business and operational structure supporting BreezeMed Urgent Care. Patients should understand that their medical care is provided exclusively by licensed healthcare professionals employed or contracted by Health Wellness Medical Astoria PLLC.
